Referent Partnership Managers
Our managers for Outreach Services are here to help you every step of the way. Whether this is your first time referring a patient or you are a longtime partner, they will work closely with you to ensure a positive experience for you and your patient. Some areas they will work closely with you on include:
- Your preference on notification is your patient admits to one of our facilities
- Your preference on notification on their discharge
- Desired level and means of communication
- Desired involvement (if any) in continuing care planning
- If you would like us to provide medical records after discharge
